In onze praktijk verwerken wij persoonsgegevens om u goede medische zorg te kunnen bieden en de administratie correct af te handelen. Dit gebeurt volgens de geldende wetgeving, waaronder de AVG en de WGBO.

Use of data
Your data is used only for:
• providing medical care;
• practice management and quality improvement;
• (if applicable) education and research.

Processing may also be necessary to comply with legal obligations or to protect your health or that of others.

Protection of your data
We handle your data with care and confidentiality:
• all staff are bound by professional confidentiality;
• your data is securely protected;
• data is not kept longer than necessary (at least 20 years according to the law).

Your rights
You have the right to:
• access your medical record;
• receive a copy of your data;
• request correction or addition;
• request (partial) deletion where legally permitted;
• add your own statement to your record;
• objection to processing in certain situations.

You can submit a request to the practice at any time.

Sharing of data
Your data is only shared with third parties with your explicit consent, unless there is a legal obligation or an emergency.
Where necessary for your treatment, data may be shared with other healthcare providers, such as pharmacists or medical specialists.

With your consent, we exchange medical data via the National Exchange Point (LSP), for example with out-of-hours GP services. We are also connected to Mitz, a national system where you can manage your consent preferences.

Transfer of medical records
If you register with a new GP, your medical record will be transferred securely and as quickly as possible (within one month), with your consent.
